  • Overview

    This position supports the patients, families and staff at the University of Minnesota Masonic Children's Hospital. Typical duties will include: assist with information and referrals to appropriate internal and external resources; trouble shoot complex issues/barriers with interdisciplinary team, assess and provide support for appropriate financial assistance and provide supportive counseling to patients and families with problems that are related to hospitalization, trauma or grief/loss. Assist with screening, identification and coordination of care with Child Protection to children being assessed for suspected abuse/neglect. 

     

    This is done primarily by pager, with some required on site visits, in the evenings, overnights, weekends and holidays. Minimum work requirement is one shift each month. A shift includes carrying a pager from Monday 4p- Friday 8a (excluding weekday hours) OR Friday 4p – Monday 8a.  One weekend around a holiday or one designated holiday is also required each calendar year.  Some work can be triaged by phone but other consults may require onsite assessment including most suspected child abuse cases. 

     

    Responsibilities/Job Description

    • Plans and implements treatment and transition care plans based upon a psychosocial assessment that is integrated into an interdisciplinary care plan
    • Provides a range of social work interventions that include casework, counseling crisis intervention, case management, and group work methods in assisting patient and families reach their goals. Identifies and recognizes rapidly changing situations and quickly intervenes to prevent or resolve the emotional crisis, and increased LOS, or unnecessary admissions
    • Arranges, mobilizes, utilizes, and may coordinate a comprehensive range of community resources which most effectively meet the identified patient and family needs
    • Takes leadership role in facilitation of interdisciplinary communication and guides team members interactions with patient/families experiencing complex psychosocial issues
    • Formally and informally educates patient/family and health care team members regarding the social work role, pertinent patient care issues, and population specific information
    • Serves as a preceptor or field instructor for social work graduate and undergraduate training as appropriate to their skills related to a range of psychosocial issues and interventions
    • Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care
    • Documents patient and family assessment, history, diagnosis, treatment and transition care plan, and progress, in the medical record
    • Identifies and participates in CQI, research and program evaluation initiatives that focus on improving health care services at a service, program. Or department level
    • Demonstrates knowledge and compliance with Fairview and Social Work Services standards, policies, and procedures
    • Complies with the legal and regulatory laws, reporting and practice standards within the practitioners scope of practice
    • Meets all role-related and age-specific competencies

    Qualifications

     

     

    Required 

    • Master’s degree in Social Work from an accredited social work program
    • Current MN Social Work license 

    Preferred 

     

    • Current MN Social Work license at the LGSW or LISW level
    • One year experience in a health care setting
    • Pedicatric experience 
    • Child Abuse/Maltreatment or Protection as well as grief/loss experience preferred
