  • Overview

    Home Care and Hospice is looking for a Hospice RN to travel throughout the metro area. This is casual position, requiring one weekend a month.

     

    Responsibilities/Job Description

    • Ongoing development and implementation of the plan of care
    • Coordination of services
    • Maintains complete, accurate, legible and timely clinical records/ documentation on all patients according to agency policy
    • Provides safe, effective skilled and advanced intervention, within scope of practice according to patient need.
    • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served
    • Fulfills all organizational requirements
    • Performs other duties as assigned

    Additional responsibilities:

    • Stays current (expert) in home care regulation and all applicable licensing
    • Actively participates in educational programs, meetings and agency committees as appropriate
    • Accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures
    • Identifies processes to improve and potential solutions

    Qualifications

    Required:

    • One year recent RN experience.
    • Computer skills: able to document effectively in Electronic Medical Record (EMR).
    • Licensed as a RN and eligible to practice in Minnesota.
    • Current BLS (for Healthcare Providers) certification.

    Preferred:

    • Bachelor’s degree in Nursing.
    • RN experience in oncology, med/surg, critical care or high tech home care/ hospice.
    • Public Health Nurse (PHN) certification.

     

    Additional Requirements:

    • Reliable transportation
    • Valid Minnesota driver’s license (or license that matches your current permanent residence)
    • Current and adequate automobile insurance policy
